The Novo Nascer Hospital instructs its employees to act in accordance with ethical principles and values that aim to offer safe, dignified and maximum privacy treatment to our patients and family members. This Declaration of Rights and Responsibilities is in accordance with Ordinance no. 1,820 of the Ministry of Health.
01. Receive adequate, dignified, attentive, respectful, humanized, welcoming service and free from any discrimination;
02. To be identified by all professionals by their civil or social name of preference, without being called inappropriately, nor be treated by numbers, codes or in a generic or disrespectful way;
03. Have their personal data kept confidential, as long as it does not pose risks to third parties or public health;
04. To have respected their physical integrity, privacy, comfort, individuality, ethical values, religious cultural values, maintaining the confidentiality of any and all personal information, security in the procedures to which they are submitted and psychological and emotional well-being;
05. Identify the people who assist them with a badge that contains their name and function;
06. Obtain clear information about your treatment, diagnostic hypotheses, tests, singular therapeutic plan, risks and benefits of the proposed treatments, duration of treatment, purpose of materials collected for tests, prescription drugs;
07. Freely consent or refuse (if voluntary resident), after clarification, diagnostic or therapeutic procedures;
08. Have their medical record updated and readable, the reason for their care and/or hospitalization, observation data and clinical evolution, therapeutic prescription, evaluations of team professionals, nursing procedures and care, results of complementary laboratory tests and radiological, identification of the person responsible for the notes and other information that may be necessary;
09. Receive medical report with the diagnosis and treatment indicated, as well as Summary of Discharge at Hospital Discharge or Medical Report at Discharge on Request;
10. Receive prescriptions typed or in legible handwriting, with the name of the professional and his registration number, signed by him and with the generic name of the prescribed medications;
11. Be accompanied in outpatient consultations, if they wish;
12. RAdequately receive from professionals, as appropriate, immediate assistance to improve their health, comfort and well-being, in a dignified and appropriate place;
13. Receive spiritual or religious assistance if they wish, without this changing the treatment routine;
14. Be previously and expressly informed if the proposed treatment is experimental or part of a research and refuse it if they wish;
15. In compliance with the Statute of the Elderly, we clarify that, due to the best therapeutic result, monitoring by a family companion is not allowed, however, assistance by a caregiver is possible, at the expense of the legal guardian, (if the team evaluates this need);
16. In compliance with the Consumer Protection and Safety Code, the patient and his family are assured basic consumer rights, such as protection of life, health, safety against risks, adequate information on treatment and services provided;
17. The provision of services is subject to the authorization of the respective operator to which the insured is linked or at the expense of the insured;
18. Be removed in a timely manner and in safe conditions to a general hospital, when necessary, together with a summary of their clinical history, list of current medication in their medical record, possible diagnoses, treatment performed, evolution and reason for referral and having their family or legal guardian immediately informed upon discharge by transfer to a hospital;
19. Decide whether their family members or guardians can be informed about specific conditions of their health;
20. Receive visits from other health professionals who do not belong to our clinical staff, giving this professional access to medical records, with fees at the expense of the patient;
21. The indication, of their free choice, to whom they will entrust decision-making in the event of becoming unable to exercise their autonomy;
22. Express and be heard in their complaints, reports, needs, suggestions and other manifestations through the Customer Service Center and Ombudsman.
01. The patients and their family must provide all information about the patient’s health, including the use of medications, drugs, allergic reactions, as well as presenting reports and exams of previously performed treatments, and any other information about their health status;
02. The patients and their family must follow the instructions of the multidisciplinary therapeutic team;
03. The patients and their family must express their understanding of the therapeutic measures proposed by the Institution, and also express their doubts about the treatment, when they exist;
04. The patients must inform the health care team about changes in their health status, including the appearance of pain;
05. The patients and their family must follow the internal rules of the Novo Nascer and also those related to the treatment program in which they are inserted, also respecting the rights of other patients and their respective family members, valuing a respectful and harmonious living environment;
06. The patients and their family must assume responsibility for the refusal of recommended procedures, exams or treatments and for non-compliance with the guidelines of the professional or the multidisciplinary team;
07. Indicate the financial responsible for your hospital treatment and, in the case of an agreement, the person responsible for any expenses not covered by the health plan;
08. Do not use drugs or alcohol in the Institution, nor offer money or any type of benefits to professionals of the Institution or other residents;
09. Use tobacco only in places where permitted;
10. Develop habits that improve their health and quality of life;
11. Ensuring the conservation of objects, furniture, electronic equipment, educational material, among others used is everyone’s duty. Any material damage caused will be charged to those responsible, as stated in the contract. Thus, the functioning of the hospital is protected and therapeutic activities develop normally;
12. Any kind of loving and sexual involvement in the hospital environment is also prohibited, one of our principles. As it is also prohibited to sell, loan or exchange personal items;
13. Keeping personal spaces always organized is also a posture of therapeutic importance. That is why we ask that each guest is responsible for the daily organization of their belongings, the closet itself, and leave the bed made upon waking up. It is also necessary to use the bathroom respecting the common rules of collective living, leaving the environment in good condition for other roommates;
14. Failure to comply with rules may generate consequences of a therapeutic nature according to the assessment of the multidisciplinary team;
15. Family members and/or residents are responsible for providing complete bedding, extra snacks if required and participate in meetings and gatherings at Novo Nascer when called upon;
16. The use of violence of any kind, including verbal, is prohibited, as well as taking photographs or videos in any of the hospital environments, even on commemorative dates. Entry with any image capture equipment is prohibited for greater security for everyone. The privacy of our patients is a non-negotiable item.
